FREE PHOEBE PHYSICALS

Today, April 18th from 1-3 PM at Sherwood Christian Academy in the Gym!
Phoebe Putney will be offering free physicals to SCA athletes. This is for all athletes: Rising 8th-12th grade.

No matter the status of your child’s current physical, now is the time to get reevaluated. We will do free physicals at this time every year.

We can’t stress enough the importance of making this date. This helps us as a staff in so many ways to be prepared for the new school year.

Please make sure you fill out the correct documents below!

Complete the 3 Forms:
-History Form
-Clearance Form
-Physical Examination Form

These are found in the team app under documents and all must be signed by parent or guardian.
