What : All Athletes must have a yearly physical exam on file. As we move into a new conference, we are updating the forms to GIAA.

When: Today, March 29th at 1:00 pm

Where: All physical examinations will be completed at -

Phoebe Orthopedics of Albany
2709 Meredyth Dr Unit 450
(Take elevator to 4th floor)

Who: Rising 8th to 12th graders.

Extra Info: Athletes will need to bring their forms with completed medical history by a parent and all documents signed by parent/guardian as well as athlete where indicated on form.

Parents are asked to remain in the lobby during physicals, please.

Cost: This is a free physical giving by Phoebe to all our athletes.
