All Athletes must have a yearly physical exam on file. As we move into a new conference we are updating the forms to GIAA.

The physicals will be at Phoebe Ortho on Meredyth Drive, my apologies as I did not make that clear in my previous communication.

They will need to bring their forms with completed medical history by a parent and all documents signed by parent/guardian as well as athlete where indicated on form.
Parents are asked to remain in the lobby during physicals, please.

Rising 8-12 graders. This is a free physical giving by Phoebe to all our athletes. Save the dates and make plans to attend.
